1 Types of Data Collected
1.1 Registration and Profile Information
You must create an account to use our Service. This may be done by providing your email address. You may also voluntarily add or edit your profile information such as phone number, address, weight, and information related to your diabetes management such as your diabetes type, glucose range, A1C level, insulin and medicines, and miscellaneous comments for Mobile Application Services and Patient Web Services. For Institute Web Services, name, address, name of institute, designation, and phone number are mandatory.
1.2 Personal Data
While using our Service, we may ask you to provide us with certain personally identifiable information that can be used to contact or identify you ("Personal Data"). Personally identifiable information may include, but is not limited to:

We may also collect information that your browser sends whenever you visit our Service or when you access the Service through a mobile device ("Usage Data").
This Usage Data may include information such as your computer's Internet Protocol address (e.g. IP address), browser type, browser version, the pages of our Service that you visit, the time and date of your visit, the time spent on those pages, unique device identifiers and other diagnostic data.
When you access the Service by or through a mobile device, this Usage Data may include information such as the type of mobile device you use, your mobile device unique ID, the IP address of your mobile device, your mobile operating system, the type of mobile Internet browser you use, unique device identifiers and other diagnostic data.

We may use both session Cookies (which expire once you close your web browser) and persistent Cookies (which stay on your computer until you delete them) to provide you with a more personal and interactive experience on our Site. Persistent Cookies can be removed by following Internet browser help file directions. At this time, we do not recognize automated browser signals regarding tracking mechanisms, which may include “do not track” instructions. 2.1 Provision of Services
The Personal Information you provide is necessary for such purposes including allowing you to set up a user account and profile, allowing the Registered Institute/Staff to access the uploaded User Information including medical and health information, providing the Services, using the user information to facilitate order booking through third party Partner pharmacies and Partner laboratories.
The personally identifiable information of the registered institutes, partner pharmacies and partner laboratories is not generated by Medworks and is provided by the registered institute, partner pharmacies and partner laboratories. Medworks displays such information on its Services without any representation or warranty on the accuracy or completeness of the information. The Company will, however, take reasonable steps to ensure the accuracy and completeness of this information.
Medworks may draw upon this Personal Information for commercial purposes in an aggregated or non- personally identifiable form for advertising including advertising by third parties, statistical analysis and business intelligence purposes, for sale or transfer of such research, statistical or intelligence data in an aggregated or non-personally identifiable form to third parties and affiliates.
Medworks may draw upon this Personal Information for academic research purposes and in an aggregated or non- personally identifiable form for research, and may sell or transfer data in an aggregated and non-personally identifiable form to accredited research organizations or institutions.
2.4 Miscellaneous Usage
Medworks may also use information provided by the Users to contact them regarding new services, solicit feedback, improving Services, and/or customizing any advertising.
3 Disclosure of Data
3.1 Vendors and Service Providers
We may employ third party companies and individuals to facilitate our Service ("Service Providers"), to provide the Service on our behalf, to perform Service-related services or to assist us in analyzing how our Services are used.
These third parties have access to your Personal Data only to perform these tasks on our behalf and are obligated not to disclose or use it for any other purpose.
We may use third-party Service Providers to monitor and analyze the use of our Service.
An example of such a service is Google Analytics. It is a web analytics service offered by Google that tracks and reports website traffic. Google uses the data collected to track and monitor the use of our Service. This data is shared with other Google services. 3.3 Affiliates and Acquisitions
Other Disclosures. Regardless of any choices you make regarding your Personal Data (as described below), Medworks may disclose Personal Data if it believes in good faith that such disclosure is necessary:
- in connection with any legal investigation to comply with relevant laws or to respond to subpoenas, court orders, warrants, or similar documents issued by a court having jurisdiction and served on Medworks;
3.4 Registered Institutes/Pharmacies/Laboratories:
Users of Institute Web Service can send requests to users of Phone Service and Patient Web Service to access their uploaded User Information, including medical and health information. Phone Service and Patient Web Service user data will only be shared with registered institutes/staff following Phone Service and Patient Web Service Users’ approval. Users of Phone Service and Patient Web Service may at any point revoke access for registered institutes/staff, thereby preventing them from seeing User data.
Registration and Profile Data to the extent of name, contact information, and address of users of Phone Service and Patient Web Service may also be shared with third parties to facilitate order booking through third party partner pharmacies and partner laboratories.
The personally identifiable information of the registered institutes, Partner pharmacies and Partner laboratories is not generated by the Company and is provided by the registered institute, Partner pharmacies and Partner laboratories. Medworks displays such information on its Services without any representation or warranty on the accuracy or completeness of the information. Medworks will, however, take reasonable steps to ensure the accuracy and completeness of this information.
3.5 Disclosure for Commercial and Academic Use
Medworks may disclose data in an aggregated and non-personally identifiable manner with partners and third-parties in accordance with Paragraphs 2.1 and 2.2
4 Voluntary Nature of Providing Information
You hereby also give your consent to the Company to store the User Information on the Company’s servers, regardless of where the servers are located.
5 Information Deletion
You can contact Medworks on firstname.lastname@example.org to inquire whether the Company is in possession of your personal data. If you do not want the Company to be in possession of such data you may require the company to delete and destroy all such information. We will make good faith efforts to make requested changes in our then active databases as soon as reasonably practicable. If you provide any information that is untrue, inaccurate, out of date or incomplete (or becomes untrue, inaccurate, out of date or incomplete), or the Company has reasonable grounds to suspect that the information provided by you is untrue, inaccurate, out of date or incomplete, the Company may, at its sole discretion, discontinue the provision of its services including the Services provided to End-Users.
5.1 Protection of Information
Medworks relies on a HIPAA-compliant cloud server for the storage of data. Strict policies are in place to prevent unauthorized access, improper use or disclosure, unauthorized modification, and unlawful destruction or accidental loss.
Medworks implements reasonable security practices and has physical, technical and administrative controls in place to minimize the risk of data breach. These practices include encryption of data in transit, storing data on HIPAA-compliant cloud servers, and regularly changing passwords. Passwords too are of substantial complexity.
It is important for you to protect against unauthorized access to your password, and Service access devices. The Company does not undertake any liability for any un-authorized use of your login to Medworks. If you suspect any unauthorized use of your login to Medworks, you must immediately notify the Company by sending an email to email@example.com. You shall be liable to indemnify the Company due to any loss suffered by it due to such unauthorized use of your login to Medworks.
The Company makes all User information accessible to its employees, agents or partners and third parties only on a need-to-know basis, and binds all such employees to strict confidentiality obligations through legal contracts.
Please note that data is stored on a cloud, and may be stored in a geography whereby best service access is available in terms of data transmission rates.
We may revise this policy from time to time. When we do, we will post the revised policy on this web page, and the revised version will be effective when it is posted. If we make any material changes to this policy, we will notify you of such changes by posting them on this page or by sending you an email or other notification, and we will indicate when such changes will become effective. By continuing to access or use our Services after those changes become effective, you are agreeing to be bound by the revised policy.
